If you require a sick note please contact our reception team on 01455 553531. If this is the first time you are requesting a sick note about this current period of illness, you will require an appointment with a GP. This can often be a telephone appointment.
Please note that we require 48 hours to process a sick note request.
GPs can refer employed patients to Fit for Work, with their consent, if they are, or are likely to be, off sick for four weeks or more.
The patient will meet with an occupational health professional who will produce a Return to Work Plan, which can replace the need for a fit note. The Return to Work Plan will be tailored to the patients’ needs to help them return to work in a way that’s right for them.
